Sports Physicals will be available at Goshen High School—All students planning to participate in a school sport and will be in grades 7-12, will need an up to date physical for the 2012-2013 sports year. For your convenience, Goshen Family Practice will be giving sports physicals at Goshen High School from 3:00-6:00 on May 23rd.
OHSAA forms that will be used and that need to be completed are available here
Cost is $10 and all of the money is donated back to the Athletic Department. Checks should be made out to Goshen Family Practice. This physical will be good for all sports next year.
